Event Report
This form is part of Cavok Air’s voluntary reporting system and is intended for reporting occurrences, hazards, concerns, or unsafe conditions that may affect aviation safety.
Reports submitted through this form are received without identifying information. Cavok Air will not know who submitted the report unless you choose to reveal your identity within the report itself. Reporting is voluntary, anonymous, and non-punitive.
What can be reported

Complete the form with as much information as possible. No personal details are required.
Identity remains unknown
The report is submitted without identifying information. Unless you include personal details within the report itself, your identity cannot be determined.
Every report is reviewed
All reports are assessed and followed through. Reports involving an immediate or significant safety risk are prioritised and escalated without delay.
Anonymous Report
What to include in your report
Include as much detail as you are able to provide. Because anonymous reports cannot be followed up directly, providing clear and complete information is particularly important.
Date, time and location
When and where the event occurred, including airport, facility, aircraft registration, or airspace if applicable.
Description of the event
What happened, what was observed, and the sequence of events. Be as factual and specific as possible.
Hazard or safety concern
What the safety implication was or could have been, and any contributing factors you identified.
This reporting system does not replace mandatory occurrence reporting requirements or your organisation’s established safety reporting procedures where these apply.
