Event Report
This form is part of Cavok Air’s voluntary reporting system and is intended for reporting occurrences, hazards, concerns, or unsafe conditions that may affect aviation safety.
Reports submitted through this form are reviewed exclusively by the Flight Safety Manager. Your identity is known only to the Flight Safety Manager and will not be disclosed without your consent. Reporting is voluntary, confidential, and non-punitive.
What can be reported

You may use this form to report any of the following:
Submit your report
Complete the form with as much information as possible. If additional clarification is required, the Flight Safety Manager may contact you directly.
Identity is protected
Your identity remains visible only to the Flight Safety Manager. Before information is shared internally for safety review, all identifying details are removed.
Every report is reviewed
All reports are assessed and followed through. Reports involving an immediate or significant safety risk are prioritised and escalated without delay.

What to include in your report
Include as much detail as you are able to provide. Incomplete reports are accepted. The Flight Safety Manager may contact you to gather additional information if needed.
This system does not replace the obligation to submit mandatory occurrence reports to your regulatory authority or to follow your organisation’s standard safety management procedures.
Date, time and location
When and where the event occurred, including airport or airspace if applicable.
Description of the event
What happened, what was observed, and the sequence of events. Be as factual and specific as possible.
Hazard or safety concern
What the safety implication was or could have been, and any contributing factors you identified.
This reporting system does not replace mandatory occurrence reporting requirements or your organisation’s established safety reporting procedures where these apply.
