This form is part of Cavok Air’s voluntary reporting system and is intended for reporting occurrences, hazards, concerns, or unsafe conditions that may affect aviation safety.
Reports submitted through this form include your identity and contact details. This enables direct communication with the Flight Safety Manager and supports a more detailed review of the event where necessary. Reporting is voluntary, open, and non-punitive.

Complete the form with as much information as possible, including your contact details.
Direct communication
The Flight Safety Manager may contact you to clarify details, gather additional information, or discuss the event further if required.
Every report is reviewed
All reports are assessed and followed through. Reports involving an immediate or significant safety risk are prioritised and escalated without delay.
Open Report
What to include in your report
Include as much detail as you are able to provide. Providing your contact details allows the Flight Safety Manager to follow up directly and gain a clearer understanding of the event if required.
Date, time and location
When and where the event occurred, including airport, facility, aircraft registration, or airspace if applicable.
Description of the event
What happened, what was observed, and the sequence of events. Be as factual and specific as possible.
Hazard or safety concern
What the safety implication was or could have been, and any contributing factors you identified.
This reporting system does not replace mandatory occurrence reporting requirements or your organisation’s established safety reporting procedures where these apply.
